The privacy of our clients’ personal information is important to us, and we have always been bound by professional standards of confidentiality. However, we are required by law to formally inform you of our privacy policy.

Legal Authority to Collect Personally Identifiable Information (PII)

Our agency provides services as a life and health insurance brokerage agency to help individuals and other entities learn about, compare, apply for, and secure insurance coverage. The types of insurance we work with include but are not limited to life, dental, health, disability, and short and long term disability coverage.

As licensed agents or brokers and as an entity permitted to assist consumers within the Federally Facilitated Marketplace (FFM, Exchange, and Marketplace) under the Affordable Care Act, we are authorized to collect certain nonpublic Personally Identifiable Information (PII) from clients and prospects.

Personally Identifiable Information (PII) is “information that can be used to distinguish or trace an individual’s identity, alone, or when combined with other personal or identifying information that is linked or linkable to a specific individual.”

Types of Personally Identifiable Information (PII) we may collect

In the course of our agency’s professional practice, we may collect PII about you, your employees, and/or dependents.  This information is provided by you or obtained by us with your consent. This information may be collected via various means (i.e. mail, fax, email, phone, in person, etc.). It may also include information we receive from personal interviews, applications and/or other documents necessary to provide you with our professional services.

The PII we collect may comprise, but is not limited to, age, date of birth, gender, social security number, marital status, Employer Tax ID (EIN), payroll records, wage and tax reports, individual medical or health status information, employer name, occupation, medical and attending physician statement(s) or records, credit information, email, phone numbers, and IP address.

Purpose for Collecting Personally Identifiable Information (PII)

The purpose for collecting PII information is to help our customers obtain eligibility determinations, compare plans, enroll in insurance coverage, and for the provision of ongoing client service.

Authorized Uses and Disclosures of Personally Identifiable Information (PII)

We use and disclose PII only as required to engage in the reasonable course of providing professional services on your behalf or as otherwise required by law.  We restrict access to personally identifiable information about our clients to the members of our firm requiring such access for the delivery of our professional services.

Voluntary or Mandatory Requests for Personally Identifiable Information (PII)

Requests for PII may be either mandatory or voluntary under applicable law or as stipulated by contract provisions and terms.

Effects of Non-Disclosure of Personally Identifiable Information (PII)

If an individual or entity chooses to non-disclose requested or required PII, it may result in adverse consequences including but not limited to inaccuracy of proposals, refusal of application, policy cancellation, loss of coverage, or policy rescission.

Ability to Access, Revoke, Update, or Modify Consent of Use of Personally Identifiable Information (PII)

Individuals and entities may access, correct, amend, substitute, or delete their PII and/or their consent for our use of their PII in accordance with applicable law.

Safeguards, Documentation, Storage, and Accounting for Personally Identifiable Information (PII)

We employ physical, electronic, and procedural security safeguards to protect our clients’ nonpublic personally identifiable information.

We retain disclosure, and consent information for PII as required by law. We retain records relating to the professional services we provide in accordance with agency, accounting and/or government standards.

Other General Privacy Policy Provisions

We collect, disclose, access, maintain, store, and use PII from individuals and other entities only for the functions and purposes listed in this Privacy Policy Notice/Statement. Unless we obtain other informed consent, we will adhere to the provisions as stipulated here.
