- Lifetime benefit limits in dollar values to any participant or beneficiary (pp 19, lines 6-7).
- Unreasonable annual limits on the dollar value of any limits for a participant or beneficiary (pp 19, lines 8-11).
Some health plans are limited in benefit and may not be required under Federal and State law to provide “essential health benefits.” Such plans will be unaffected by this rule (pp 19, lines 12-19).
